Job summary
Rocky Mountain PACE seeks a physician to provide community-based rounding in subacute SNFs, leading clinical oversight within a dedicated interdisciplinary team. You will mentor NPs and other providers while championing person-centered care for frail elderly participants.
Competitive salary commensurate with experience, with a $5,000 annual CME allowance and a flexible schedule offering strong work-life balance. Health, dental, vision, and retirement benefits are included.
Qualifications
- MD or DO licensed to practice in Colorado.
- ABMS Board Certification in Internal Medicine or Family Medicine (geriatrics preferred).
- DEA registration; ability to maintain hospital privileges.
- Experience in primary care with frail or elderly populations.
- Passion for mission-driven, whole-person care.
Responsibilities
- Community-based physician rounding in subacute skilled nursing facilities.
- Lead clinical oversight and quality for PACE provider services.
- Collaborate with an interdisciplinary team.
- Serve as physician leader and mentor to NPs and other providers.
- Champion person-centered care for frail elderly participants.
