Job summary

Curative's PACE program in the Greater Denver Region seeks a compassionate Advanced Practice Provider (NP/PA) to deliver long-term geriatric care with an emphasis on relationship-building and holistic management. Enjoy a low-volume schedule, a dedicated remote admin day, and leadership access without RVU quotas.

You will work within an Interdisciplinary Team to provide on-site visits, community and home-based care, and remote charting with stable compensation and call stipends.

Qualifications

• Active Colorado NP or PA license and DEA certification.
• AGPCNP, FNP, ACNP or NCCPA certification.
• 2+ years experience in geriatrics, internal medicine, family practice, SNF or home care.

Responsibilities

• Manage a mature panel of 75-80 seniors with a gradual ramp-up.
• Clinic visits, acute assessments, and daily IDT meetings on-site Mon-Wed.
• Community visits on Thu; remote charting and care coordination on Fri.
• Report to Medical Director with limited RVU pressure and call pay.

Our Program of All-Inclusive Care for the Elderly (PACE) center in the Greater Denver Region is looking for a compassionate, skilled Advanced Practice Provider (NP or PA) to join our collaborative Interdisciplinary Team (IDT).

Requirements

• Active (or eligible for expedited) Colorado NP or PA license & DEA certification.
• AGPCNP, FNP, ACNP, or NCCPA certification.
• 2+ years experience in geriatrics, internal medicine, family practice, SNF, or home care preferred (standout new grads with strong RN/rotation background considered).
