Theoria Medical is seeking a part-time Medical Director and Attending Physician to provide high-quality primary care at multiple skilled nursing facilities. The role emphasizes autonomy over scheduling, collaborative clinical support, and a patient-centered approach to care.

Responsibilities

• Serve as the attending physician for patients in each assigned SNF.
• Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
• Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
• Participate in chronic care management, transitional care, remote monitoring, and other quality initiatives.
