Job summary

Theoria Medical is seeking a full-time Medical Director and Attending Physician to provide medical leadership and primary care at local skilled nursing facilities. You will serve as the attending physician for SNF residents, collaborate with NPs/PAs, supervise clinical care, and participate in chronic care management, remote monitoring, and quality initiatives.

Qualifications

• Must hold an active U.S. medical license and be board-certified in an appropriate specialty.
• New graduates who are board-eligible are encouraged to apply.
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, Geriatric Medicine.

Responsibilities

• Serve as attending physician for SNF patients at assigned facilities.
• Collaborate with NPs/PAs to coordinate daily patient care and supervision.
• Provide high-quality primary care, diagnosing and treating acute/chronic conditions.
• Participate in chronic care management, transitional care, and quality initiatives.
