Qualifications

• Active U.S. medical license and board-certified in a relevant specialty.
• New graduates who are board-eligible and pursuing licensing are encouraged to apply.
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

Responsibilities

• Serve as the attending physician for patients in each assigned SNF.
• Collaborate with NPs/PAs to manage daily patient care and supervise clinical teams.
• Provide high-quality, patient-centered primary care including diagnosing and treating acute/chronic conditions.
• Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring and other quality initiatives.
