Theoria Medical is seeking a full-time Medical Director and Attending Physician to serve local skilled nursing facilities in a mission-driven care community.

You will lead patient care, collaborate with NPs/PAs, and enjoy unmatched schedule flexibility, a supportive clinical team, and access to technology that streamlines documentation and care.

Formación

• MD/DO with active U.S. medical license and board certification.
• Board-certified in Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, or Geriatric Medicine.
• New graduates who are board-eligible and actively pursuing licensing are encouraged to apply.

Responsabilidades

• Serve as the attending physician for patients in each assigned SNF.
• Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
• Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
• Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
