Theoria Medical is seeking a full-time Medical Director and Attending Physician to serve local skilled nursing facilities within a mission-driven care community. Qualifications

• MD/DO with active U.S. medical license and board certification
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine
• New graduates board-eligible encouraged to apply

Responsibilities

• Serve as the attending physician for patients in each assigned SNF.
• Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
• Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
• Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
