Job summary

Theoria Medical is seeking a Medical Director and Attending Physician to cover local skilled nursing facilities. You will oversee patient care as the attending physician, collaborate with NP/PA teams, and drive quality initiatives in a flexible, physician-centric model.

Qualifications

• Active U.S. medical license required.
• Board-certified in an appropriate specialty; board-eligible candidates encouraged.
• MD/DO degree from an accredited program.

Responsibilities

• Serve as attending physician for residents in SNFs.
• Collaborate with NPs/PAs to manage daily patient care and supervision.
• Provide high-quality, patient-centered primary care, diagnosing and treating acute and chronic conditions.
• Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and related quality initiatives.
