Job summary

CommuniCare Health is seeking a Medical Director to lead a new model of care in post-acute and long-term settings. The role blends bedside patient care with innovative technologies to reduce readmissions and improve outcomes.

You will oversee medical services, ensure regulatory compliance, coordinate with physicians, and provide services in skilled nursing facilities or clinics. This leadership position requires a strong geriatrics background and experience guiding NP teams.

Qualifications

• MD or DO degree required.
• Ohio medical license required.
• ABMS board certification in Internal Medicine, Family Practice, Geriatrics or Hospice/Palliative.
• Gerontology certification preferred.
• 5+ years experience with geriatric or long-term care populations.
• Experience educating nurse practitioners.

Responsibilities

• Assume administrative authority to implement medical services, policies and procedures in post-acute and long-term care settings.
• Coordinate medical care, liaise with attending physicians, and ensure quality of care remains under surveillance; deliver physician services in facilities.

Job Duties and Responsibilities

As Medical Director, you will be part of an exciting new model of care combining good old fashioned bedside manner with utilizing innovative technologies to dramatically reduce patients' return to hospital and achieve better outcomes. Job Duties and Responsibilities: Assume the administrative authority, responsibility, and accountability of implementing our medical services, policies, and procedures according to federal, state and local regulations; and current practice of post-acute and long-term care medicine. Coordinate medical care, including emergency treatment procedures, maintain effective liaison with attending physicians, and implement methods to keep the quality of care under constant surveillance. Provide necessary physician services for those residents within the skilled nursing facilities and/or clinic environment

Qualifications

• M.D. or D.O. required
• Gerontology certification preferred
• MD or DO license in State of Ohio
• ABMS Board Certified in: Internal Medicine, Family Practice, Geriatrics or Hospice/ Palliative
• 5+ years of experience working with geriatric or long-term care population
• Demonstrated experience working with and educating NP’s

Preferred Qualifications

• Health care management/leadership experience
• Familiarity with CMS, Medicare Advantage, SNP Rules and Regulations HCC and Risk Adjusted reimbursement methodology
• Inpatient/SNF utilization metrics and bed day management
• State specific collaborative practice agreements and midlevel practitioner licensing
• Knowledge of EMR
