Community Options, Inc. in Wrightsville, PA is hiring a Full-Time Medical Coordinator to plan, track, and ensure that all clients’ needs are met within appropriate time frames, including scheduling annual, follow-up, and emergency medical appointments in coordination with medical staff and program directors.

The role requires strong communication, documentation, and crisis-management skills, along with meeting license and training requirements; it supports Lancaster, PA.

Qualifications

• High school diploma or GED; bachelor’s degree preferred.
• Valid driver’s license with a satisfactory driving record.
• Complete all state and agency required training per state guidelines.
• Ability to establish a comfortable and supportive relationship with individuals receiving supports.
• Ability to exercise good judgment and remain calm in crisis situations.
• Excellent verbal and written communication skills.

Responsibilities

• Schedule all client medical appointments to state mandated licensing regulations.
• Observe and monitor the client’s status through paperwork and follow-up appointments.
• Observe and track clients' physical care including, but not limited to, annual and emergency appointments.
• Provide accurate, timely, and consistent documentation of client medical appointments in the appropriate records.
• Attend all pertinent medical appointments that need to be addressed more clearly and precisely to achieve the best results for the individual.
• Communicate with supervisors, co-workers, and medical professionals’ regarding clients’ status.
• Additional tasks and responsibilities may be assigned.
• Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation.
• Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing.
