Job summary

Community Options, Inc. is seeking a Full-Time Medical Coordinator in Camp Hill, PA. The role plans, tracks, and ensures that clients’ medical needs are met within appropriate timeframes, coordinating annual, follow-up, and emergency appointments with medical staff and program leadership.

The position requires a high school diploma or GED, valid driver’s license, and the ability to document accurately. Starting pay is $24.00 per hour, with full-time hours in Camp Hill, PA.

Qualifications

• High school diploma or GED; bachelor’s degree preferred.
• Valid driver’s license with a satisfactory driving record.
• Complete all state and agency required training per state guidelines.
• Ability to establish a comfortable and supportive relationship with individuals receiving supports.
• Ability to exercise good judgment and remain calm in crisis situations.
• Excellent verbal and written communication skills.

Responsibilities

• Schedule all client medical appointments to state mandated licensing regulations.
• Observe and monitor the client’s status through paperwork and follow-up appointments.
• Observe and track clients' physical care including, but not limited to, annual and emergency appointments.
• Provide accurate, timely, and consistent documentation of client medical appointments in the appropriate records.
• Attend all pertinent medical appointments that need to be addressed more clearly and precisely to achieve the best results for the individual.
• Communicate with supervisors, co-workers, and medical professionals’ regarding clients’ status.
• Additional tasks and responsibilities may be assigned; cooperate with license and department staff during inspections, inquiries, or investigations.
• Employment is contingent upon successful completion of background checks and drug testing.
