Community Options, Inc. is seeking a Medical Coordinator in Cicero, NY. The role will plan, track, and ensure client needs are met within required time frames, including scheduling annual, follow-up, and emergency medical appointments.

Qualifications

• High school diploma or GED; bachelor’s degree preferred.
• Valid driver’s license with a satisfactory driving record.
• Ability to establish a supportive relationship with individuals receiving supports.
• Excellent verbal and written communication skills.

Responsibilities

• Schedule all client medical appointments to state mandated licensing regulations.
• Observe and monitor the client’s status through paperwork and follow-up appointments.
• Observe and track clients' physical care including annual and emergency appointments.
• Provide accurate, timely, and consistent documentation of client medical appointments in the records.
• Attend medical appointments to address needs and achieve the best results for the individual.
• Communicate with supervisors, co-workers, and medical professionals about clients’ status.
• Cooperate with license and department staff during inspections or investigations.
• Employment is contingent upon background checks and drug testing.
