Community Options, Inc. is hiring a Medical Coordinator in Allentown, PA. The role focuses on planning, tracking, and ensuring timely fulfillment of clients' medical needs, including scheduling annual, follow-up, and emergency visits, while coordinating with medical professionals and program staff.

Qualifications

• High school diploma or GED; bachelor's degree preferred
• Valid driver's license with satisfactory driving record
• Complete all state and agency required training per state guidelines
• Ability to establish a supportive relationship with individuals receiving supports
• Ability to stay calm in crisis situations
• Excellent verbal and written communication skills

Responsibilities

• Schedule all client medical appointments to comply with state licensing regulations
• Observe and monitor client status through paperwork and follow-up appointments
• Track clients' physical care including annual and emergency appointments
• Document client medical appointments accurately and timely
• Attend pertinent medical appointments to ensure clear, precise outcomes
• Communicate with supervisors and medical staff regarding clients' status
• Cooperate with license and department staff during inspections or inquiries
• Assist with checks of criminal background, central registry, child abuse registry, and drug testing

Education

High school diploma or GED; bachelor’s degree preferred
