Job summary

Community Options, Inc. is seeking a Medical Coordinator in Cicero, NY. This role plans, tracks, and ensures clients’ needs are met within appropriate time frames and schedules all medical appointments, communicating with medical staff and program teams.

The position requires a valid driver’s license, a high school diploma or GED (bachelor’s degree preferred), and strong communication skills. Employment is contingent on background checks and drug testing.

Qualifications

• High school diploma or GED; bachelor’s degree preferred.
• Valid driver’s license with a satisfactory driving record.
• Excellent verbal and written communication skills.
• Ability to establish a comfortable and supportive relationship with individuals receiving supports.
• Ability to exercise good judgment and remain calm in crisis situations.

Responsibilities

• Schedule all client medical appointments to state mandated licensing regulations.
• Observe and monitor the client’s status through paperwork and follow-up appointments.
• Observe and track clients' physical care including annual and emergency appointments.
• Provide accurate, timely, and consistent documentation of client medical appointments in the appropriate records.
• Attend all pertinent medical appointments that need to be addressed more clearly and precisely to achieve the best results for the individual.
• Communicate with supervisors, co-workers, and medical professionals regarding clients’ status.
• Additional tasks and responsibilities may be assigned.
• Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation.
• Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing.
