Job summary

Community Options, Inc. is seeking a Medical Coordinator in Cicero, NY. This role plans, tracks, and ensures clients’ needs are met within appropriate time frames and schedules all medical appointments, communicating with medical staff and program teams.

The position requires a valid driver’s license, a high school diploma or GED (bachelor’s degree preferred), and strong communication skills. Employment is contingent on background checks and drug testing.

Qualifications

Responsibilities

• Schedule all client medical appointments to state mandated licensing regulations.
• Observe and monitor the client’s status through paperwork and follow-up appointments.
• Observe and track clients' physical care including annual and emergency appointments.
• Provide accurate, timely, and consistent documentation of client medical appointments in the appropriate records.
• Attend all pertinent medical appointments that need to be addressed more clearly and precisely to achieve the best results for the individual.
• Communicate with supervisors, co-workers, and medical professionals regarding clients’ status.
• Additional tasks and responsibilities may be assigned.
• Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation.
• Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing.

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 11 states.

We are seeking a Medical Coordinator in Cicero, NY. The Medical Coordinator will plan, track, monitor, and ensure that all clients’ needs are being met within appropriate time frames. The Medical Coordinator will be responsible for scheduling all annual, follow-up, and emergency medical appointments. This additionally includes communicating with medical professional staff, program specialists, supervisors, and program directors.

Starting Pay Rate: $19.00 per hour

Schedule: Tuesday - Saturday

• Must have a valid Driver's License*
