Job summary

Signature Performance is seeking an experienced medical coder to assign principal and secondary diagnoses and procedures from medical records, ensuring accurate coding for DRG/APC and compliant billing.

You will review multiple sources in the chart, conform to Official Coding Guidelines, and communicate with clinicians to clarify documentation. Minimum 2 years of professional coding, AHIMA/AAPC certifications, and experience with EHR systems required.

Qualifications

• Minimum 2 years of Medical Coding experience required.
• Experience with Professional Fee Coding.
• Experience with EHR systems.
• AHIMA or AAPC certifications: RHIT, RHIA, CCA, CCS, CCS-P, CPC.

Responsibilities

• Assign principal and secondary diagnoses and procedures by reviewing documentation in the medical record.
• Review discharge summaries, history and physical, progress notes, radiology, labs, pathology, operative reports to assign codes.
• Assigns and ensures correct code selection following Official Coding Guidelines and compliance with regulations.
• Sequence diagnoses and procedures to accurately assign DRG, APC or payment tier.
• Review coding for accuracy and completeness prior to submission to billing.
• Abstract medical and demographic information and enter data into the system; correct errors after review.
• Identify incomplete or conflicting documentation and formulate physician queries for clarification.
• Meet coding quality and quantity expectations.
