Job summary

Chicanos Por La Causa, Inc. is seeking a bilingual Medical Case Manager for our LUCES team in Phoenix to lead HIV education and care efforts. This full-time role runs Mon-Fri, 8 am–5 pm, at 6850 W. Indian School Rd. The position supports clients in both Spanish and English and offers a comprehensive benefits package.

You will manage patient caseloads, conduct needs assessments, coordinate referrals, and collaborate with community partners to improve health outcomes and access to services.

Qualifications

  • Bachelor’s degree in a licensed field related to case management preferred.
  • Or high school diploma with 4 years of medical case management experience.
  • Knowledge of HIV/AIDS and ability to educate clients in English and Spanish.
  • Must be able to communicate with Spanish-speaking clients.

Responsibilities

  • Provide direct services to clients living with HIV and coordinate with community support.
  • Manage appointment schedules and assigned caseload as a clinical liaison to medical services.

CPLC LUCES (Latinos Unidos Contra el Sida/ Latinos United Against AIDS) provides HIV prevention education and supportive services to people living with or at risk of HIV/AIDS to improve health outcomes, reduce health disparities, and address barriers to treatment.

SCHEDULE:

Fulltime, Mon-Fri, 8 am - 5pm

LOCATION:

6850 W. Indian School Rd, Phoenix

PAY:

$26.92- $29.16 hourly, non-exempt DOE

HOW WILL YOU MAKE AN IMPACT IN THIS ROLE?
  • You will provide direct covered services to clients living with an HIV diagnosis, coordinate with community support and stakeholders.
  • You will be responsible for managing appointment schedules, assigned caseload as a clinical liaison to medical services, conducting needs assessments, implementing treatment plans, facilitating referrals, and participating in ongoing learning experiences.
  • You will provide services in both Spanish and English, helping clients navigate systems and barriers to access.

This position requires the ability to work a flexible schedule as needed per program requirements, including driving out into the community to meet clients where they are.

Minimum Qualifications And Competencies
  • Bachelor’s degree from an accredited college in a licensed* field related to case management such as social work, nursing, public health or other human services related field preferred.
  • Or high school diploma and comparable professional knowledge, skills and abilities that documents 4 years of experience specific to medical case management (conducting needs assessments, implementing treatment plans, facilitating referrals) may be substituted for the degree.
  • Knowledge of HIV/AIDS and skilled at educating clients about HIV/AIDS and other STD’s.
  • Bilingual: English/Spanish skills is a requirement. Must be able to communicate to Spanish speaking clients; considering the importance of knowing & communicating medical terminology in both languages.
  • Able to work a flexible schedule as needed per program requirements.
  • Must successfully complete the agency’s orientation and demonstrate competency, knowledge, and skills.
  • Have good organization and time management skills.
