Chicanos Por La Causa, Inc. is seeking a bilingual Medical Case Manager for our LUCES team in Phoenix to lead HIV education and care efforts. This full-time role runs Mon-Fri, 8 am–5 pm, at 6850 W. Indian School Rd. The position supports clients in both Spanish and English and offers a comprehensive benefits package.
You will manage patient caseloads, conduct needs assessments, coordinate referrals, and collaborate with community partners to improve health outcomes and access to services.

This position requires the ability to work a flexible schedule as needed per program requirements, including driving out into the community to meet clients where they are.
