Community Health Network is seeking a Medical Assistant to support pre-visit planning for Annual Wellness Visits and to update patient charts in the EMR with medical, surgical, and family histories, allergies, and medications.

The role involves collecting Health Risk Assessments, Social Determinants of Health data, and Depression Screenings, while reviewing care gaps and ensuring seamless communication with providers and staff for timely care delivery.

Formación

• Diploma from a Medical Assistant program required.
• Medical Assistant Certification/Registration is required. Certifications listed: CMA (AAMA), NCMA (NCCT), RMA (AMT), CCMA (NHA), NRCMA (NAHP).
• Will be part of four teams: MA Afternoon Inbasket, PVP, FMLA, Clinical Practice Improvement.
• Navigate PVP queue, complete chart reviews, interview patients, and document visits.
• Prepare patients for provider visits and document in the EMR.
• Obtain external lab/procedure results and communicate with staff for problem resolution.

Responsabilidades
