Community Health Network is seeking a Medical Assistant to support Pre Visit Planning and AWV workflows in our Indiana clinics. You will update patient charts, collect health histories, allergies, immunizations, medications, and complete Health Risk Assessments to help close health gaps and improve patient access.

In this role, you will work across four teams—MA Afternoon Inbasket, PVP, FMLA, and Clinical Practice Improvement—ensuring accurate documentation and timely communication with

Qualifications

• Diploma from a Medical Assistant program required.
• Medical Assistant Certification/Registration is required (CMA, NCMA, RMA, CCMA, or NRCMA).
• Ability to navigate multiple teams and clinical workflows.

Responsabilités

• Support Pre Visit Planning for AWV.
• Update patient charts in EMR with health history, allergies, medications, and screenings.
• Complete Health Risk Assessments, Social Determinants of Health, and Depression Screenings.
• Review health care gaps and coordinate related records.
• Engage with patients remotely to prepare for visits and document in the chart.
• Collaborate with MA teams: MA Afternoon Inbasket, PVP, FMLA, Clinical Practice Improvement.
• Communicate with providers and office staff to ensure issues are resolved.
