Qualifications

• High School Diploma or GED.
• BLS through American Heart Association.

Responsabilités

• Assess patient's physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records & clinical data, and evaluate patient's response and report to the responsible provider to ensure optimal patient outcome.
• Demonstrate competency in therapeutic and diagnostic interventions including vital signs, minor procedures, prevention services, previsit planning and postvisit follow up, medication refills per protocol for existing treatment plans and ensure patient flow through the clinic is efficient.
• Contributes to development and implementation of a patient plan of care with the interdisciplinary team; identify barriers to active participation and self-management of health conditions for coordinated care.
• Successful completion of all Medical Assistant Core Skills and specialty competencies as defined in the Scope of Practice for Medical Assistants.
• Maintaining patient charts and records confidentially, including importing and communicating scanned documents.
• Document ongoing status, interventions, patient response and outcomes accurately and timely.
