Luminis Health, Inc. is seeking an Advanced Medical Assistant to support the Primary Care Team.

This role coordinates patient care, provides outreach and education, and helps identify barriers to treatment goals to reduce unnecessary utilization of ER/urgent care. Responsibilities include pre-visit planning, patient education, follow-up on medications and tests, and collaboration with nurses, clinicians, and care management teams using EPIC registries and CRISP portals.

Qualifications

• Minimum of High School Diploma
• Completion of accredited medical assistant training program preferred.
• Three years of experience as a Medical Assistant
• Experience with Microsoft Office and Excel.
• Strong customer service skills
• Basic math skills.

Responsibilities

• Coordinate patient care within the care team.
• Provide patient outreach and education.
• Maintain access to Value Based Payor portals and care gap reports.
• Support pre-visit planning for Wellness Visits.
• Schedule follow-up appointments and ensure adherence to treatment plans.
