Qualifications

• High school diploma or equivalent required.
• Coursework in social work, sociology, gerontology, human services, or related field.
• Bachelor’s degree in Human Services, Social Work, Public Administration, Business Administration, or related field preferred.
• Strong communication, relationship-building, and organizational skills.
• Valid driver’s license and ability to operate a vehicle.

Responsibilities

• Identify and recruit service providers and community resources.
• Coordinate provider enrollment and onboarding activities.
• Provide training, technical assistance, and ongoing support to providers.
• Conduct provider monitoring and quality assurance reviews.
• Maintain accurate documentation and records.
• Investigate reports of abuse, neglect, exploitation, fraud, or other concerns involving provider services.
