Job summary

Qualifications

• Baccalaureate degree in human services, education or health/medical or RN license (Nebraska).
• Two years of professional experience in long-term care, gerontology or related fields.
• Ability to understand and apply state and federal regulations relevant to Medicaid.

Responsibilities

• Coordinate home and community based services using a participant-centered approach.
• Conduct comprehensive assessments to identify client strengths and needs.
• Plan, implement, and monitor service coordination with providers and family.
• Maintain HIPAA confidentiality and accurate documentation.
