Job summary

Eastern Nebraska Office on Aging (ENOA) seeks a Services Coordinator to determine and monitor long-term care needs for clients aged 65 and older receiving home and community-based services. You will coordinate services using a participant-centered approach, assess strengths and needs, and support choices for safe independent living.

The role requires a comprehensive assessment, collaboration with diverse partners, and tracking of service delivery across the Nebraska Medicaid program.

Qualifications

• Bachelor's degree or higher in human services, education or health-related field.
• Registered Nurse license in Nebraska or eligibility for licensure.
• Strong communication and documentation skills.
• Ability to work independently and in team environments.
• Proven ability to manage caseloads and coordinate services.

Responsibilities

• Perform comprehensive assessments to identify client strengths and needs.
• Coordinate home and community-based services for eligible participants.
• Provide options for safe, independent living and consumer-directed services.
• Maintain HIPAA confidentiality and adhere to applicable laws and policies.
• Develop and monitor individualized service plans with clients and caregivers.
• Travel across the five-county service area to coordinate care as needed.
