Job summary

South-Alabama-Regional-Planning-Commission seeks a Medicaid Waiver Case Manager to implement the HCBS program across SARPC. You will establish referrals, conduct on-site assessments, and develop care plans with clients and caregivers to fit needs.

Responsibilities include reviewing contractor billings, performing annual redeterminations, and preparing required reports while ensuring compliance with Alabama Medicaid guidelines. Driving is required.

Qualifications

• Bachelor's Degree from an accredited four-year college in Social Work, Behavioral Sciences, Psychology, Geriatric Studies, or related field.
• At least three months of work experience in a social work or related program.

Responsibilities

• Establish referral sources by disseminating information to organizations, agencies, and individuals.
• Conduct on-site assessments to enroll eligible Medicaid recipients in the program and follow up on status.
• Authorize contractors to initiate services after approval, and share care plans with contractors.
• Develop, monitor, and revise client care plans; conduct monthly home visits and document findings.
• Check monthly billings for accuracy and ensure alignment with care plans.
• Conduct yearly redetermination assessments and complete HCBS forms as required.
• Prepare and maintain HCBS Program Case Management Activity Records and Daily Time Logs.
• Exchange information with Medicaid Waiver clerical staff to assign tasks and follow up.
• Other duties as assigned.
