South Alabama Regional Planning Commission seeks a Team Lead to supervise Case Managers, Personal Choices Counselors, and Initial Assessment staff within the Medicaid Waiver Home and Community Based Services program. The role ensures adherence to Medicaid Waiver guidelines and timely completion of plans, visits, and paperwork.

You will coordinate weekly huddles, monitor performance, and provide guidance while maintaining quality files and reporting.

Qualifications

• Knowledge of social work programs and processes.
• Familiarity with Medicaid Waiver guidelines and home and community based services.
• Ability to supervise and manage staff.
• Good organizational and time management skills.

Responsabilités

• Supervise a team of Case Managers and Personal Choices Counselors within the SARPC area.
• Ensure compliance with Medicaid Waiver guidelines and service plans.
• Review reporting, provide feedback, and maintain tracking records.
• Conduct team meetings and address personnel issues as they arise.
• Assist with on-site assessments and enrollment processes as needed.
• Prepare and submit required reports and maintain documentation.
