CVS Health Corporation’s Aetna Medicaid Care Management Member Connections Team in Baton Rouge, LA is hiring a Care Management Associate to coordinate healthcare services by telephonic outreach and enrollment, linking members to care managers through warm transfers.

You will perform proactive outreach, document calls in the electronic record, and work in a fast-paced, team-based environment, meeting daily targets while maintaining privacy and compliance.

Qualifications

• 1-2+ years strong customer service skills coordinating service delivery with attention to customers and privacy.
• Completes documentation of each member call in the electronic record with detailed actions for compliance.
• Works independently and meets deadlines while maintaining a caring presence telephonically.
• Able to participate in a multi-disciplinary team including internal and external participants.
• Effective communication, telephonic and organizational skills; manages multiple priorities with enthusiasm.
• Meets daily metrics with speed, accuracy and a positive attitude.

Responsibilities
