Job summary

Humana Inc. seeks a Care Coordinator to support Illinois Medicaid Members within Humana Gold Plus Integrated. You will assess needs, develop personalized care plans, and connect individuals and families with resources to improve health and well-being.

You will visit members in homes and facilities, coordinate services, and monitor progress toward outcomes. A Bachelor’s degree or applicable licensure plus 1+ year of healthcare/case management experience is required.

Qualifications

• The Care Coordinator must meet one (1) of the following requirements.
• Must reside in the state of Illinois.
• 1+ years of experience in health care and/or case management.
• 1+ years of experience with Medicare & Medicaid recipients and/or long-term care and/or Home and Community based services and/or working with managed care organizations.
• Intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook.
• Experience using a variety of electronic information applications/software programs including electronic medical records.

Responsibilities

• Visit Medicaid members in homes, facilities, and other care settings with 75%-90% local travel.
• Assess care needs to ensure services in least restrictive setting.
• Identify and resolve barriers hindering effective care.
• Plan and implement interventions to meet care needs.
• Coordinate services and monitor the case management plan against personal goals.
• Monitor patient care to ensure progression toward outcomes.
