This field position emphasizes home visits and 75-90% local travel, coordinating services, monitoring progress, and assisting with grievances and appeals as needed. A Bachelor's degree or applicable licenses plus healthcare experience are required.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or related field.
• Unrestricted LPN in Illinois with one year experience conducting comprehensive assessments and provision of formal service for the elderly.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).

Responsibilities

• Visit Medicaid members in homes or care facilities with 75%-90% travel.
• Assess care needs and ensure least restrictive setting for well-being.
• Identify and resolve barriers to effective care.
• Plan and implement interventions to meet care needs.
• Coordinate services and monitor the case management plan.
• Ensure progress toward outcomes via assessment and care planning.
• Assess enrollee needs via telephonic and face-to-face evaluations.
• Assist enrollee to remediate gaps in care and access.
• Assist enrollee with grievances and appeals.
• Provide information on MCO requirements, services, and benefits.
• Refer to community resources and guide families to appropriate care resources.
