Job summary

Humana Inc. is seeking a Care Coordinator for Illinois Medicaid Members. You will assess needs, develop personalized care plans, and connect members with resources, reporting to the Manager, Care Management. The role supports MLTSS or LTSS based on business needs.

Responsibilities include home visits, telephonic and face-to-face assessments, coordinating services, and assisting with grievances and appeals. Travel will be 75-90% in local areas as part of a field position.

Qualifications

• The Care Coordinator must have a Bachelor’s degree in social sciences, social work, human services, or a related field.
• Unrestricted LPN in Illinois with one year experience in assessments and elderly services.
• Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) is required.
• Must reside in Illinois and have 1+ year health care/case management experience; familiarity with Medicare/Medicaid and long-term care is preferred.

Responsibilities

• Visit Medicaid members in homes, facilities, or other care settings; 75–90% travel.
• Assess care needs and ensure services in least restrictive settings.
• Identify and resolve barriers hindering effective care.
• Plan and implement interventions to meet needs and goals.
• Coordinate services and monitor care plans against member goals.
• Monitor progress toward outcomes via assessments, conversations, and care planning.
• Assess enrollees via telephonic and face-to-face evaluations to coordinate services.
• Assist enrollees in remediating gaps in care and access.
• Assist with grievances and appeals; provide information on MCO requirements and benefits.
• Refer enrollees to community resources and guide families toward appropriate care resources.

Education

Bachelor's degree in social sciences, social work, human services, or a related field
