Qualifications

• Bachelor's degree in social sciences, social work, human services, or a related field.
• LPN license in Illinois with 1+ year experience conducting comprehensive assessments and formal service provision for the elderly.
• Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW).
• Must reside in Illinois; 1+ year healthcare/case management experience.
• 1+ year experience with Medicare & Medicaid recipients and/or long-term care and/or Home and Community based services and/or managed care exposure.
• Intermediate to advanced computer skills; experience with Word, Excel, Outlook; EMR knowledge.

Responsibilities

• Visit Medicaid members in homes, Assisted Living Facilities, and Long-Term Care Facilities; 75%-90% travel.
• Assess care needs and ensure services in least restrictive setting.
• Identify and resolve barriers hindering care.
• Plan and implement interventions to meet care needs.
• Coordinate services, monitor and evaluate the care plan.
• Monitor patient progress toward outcomes via assessments and care planning.
• Assess enrollee needs via telephonic and face-to-face evaluations; coordinate services.
• Assist enrollee with grievances and appeals.
• Provide information on MCO requirements, services, benefits, and community resources.
• Guide members toward resources for their care; coordinate with providers and interdisciplinary teams.
