Job summary

Northwell Career Site seeks an experienced registered nurse to manage age-specific patient care using the nursing process across assessment, planning, implementation and evaluation. The role requires collaboration with physicians and health-team members, leadership in coordinating care, and directing activities of the patient care unit team.

The responsibilities include assessing physiologic and psychosocial health, forming care plans, implementing and evaluating care, and guiding discharge

Qualifications

• Graduate from an accredited School of Nursing.
• Associate’s Degree in Nursing is required.
• Bachelor’s Degree in Nursing is preferred.
• Current license to practice as a Registered Professional Nurse in NYS and/or CT.
• BLS certification required.

Responsibilities

• Assess patient physiologic health status and document findings.
• Assess psychosocial health status and support needs.
• Identify measurable patient outcomes and goals.
• Formulate and prioritize the plan of care with interdisciplinary team.
• Implement the plan with safety, accuracy, and timely communication.
• Evaluate care effectiveness and outcomes.
• Collaborate with physicians and care team; delegate appropriately.
• Provide grade I–IV decubitus skin care as needed.
• Administer medications safely and educate patients.
• Educate patient and family; coordinate discharge planning.
• Perform related duties as required.
