Job summary

Northwell Career Site is seeking a registered nurse to perform age-specific planning of care using the nursing process, including assessment, diagnosis, outcomes, planning, implementation and evaluation. You will collaborate with physicians and health team members to coordinate procedures and treatments, demonstrate leadership, clinical judgment, and delegation on the patient care unit.

Qualifications

• Graduate from an accredited School of Nursing.
• Associate’s Degree in Nursing is required.
• Bachelor’s Degree in Nursing is preferred.
• Minimum 1-2 years of professional nursing experience in the related specialty.
• Current license to practice as a Registered Professional Nurse in New York State and/or Connecticut is required.
• BLS required, plus specialized certifications as needed.

Responsibilities

• Assesses patient’s physiologic health status and documents plan of care.
• Assesses patient’s psychosocial health status and identifies needs for support services.
• Develops patient outcome criteria and individualized goals.
• Plans care with prioritized activities and coordinates resources.
• Implements the plan and administers prescribed treatments with safety and accuracy.
• Evaluates care and measures patient outcomes for quality improvement.
• Collaborates with physicians and care team; directs/delegates tasks as needed.
• Performs grade I-IV Decubitus care and skin care procedures.
• Educates patient and family about conditions, treatments, and post-discharge planning.
• Participates in discharge planning and referrals as appropriate.
• Performs related duties as required.
