Job summary

Qualifications

Responsibilities

• Develop and adjust individualized care plans.
• Administer medications safely following orders.
• Educate patients and families on conditions and self-care.
• Coordinate with staff to optimize patient care and documentation.
• Maintain accurate medical records and monitor outcomes.
