Job summary

Signature Home Health & Hospice in Bend, Oregon is seeking a Registered Nurse to provide in-home health and hospice care. You will assess patients, develop care plans, deliver skilled nursing services, and educate families while collaborating with a multidisciplinary team.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in the state of practice.
• Current CPR certification.
• Two years of nursing experience preferred; home health, public health, or hospice experience preferred.
• Excellent assessment, communication, and clinical skills.
• Ability to work independently and manage priorities effectively.
• Valid driver’s license, reliable transportation, and automobile insurance.
• Ability to perform the physical requirements of patient care including lifting, transferring, bending, standing and walking for extended periods.

Responsibilities

• Plan, coordinate, and deliver quality home health and hospice care in patients’ homes.
• Assess patient needs and develop individualized care plans.
• Provide skilled nursing services and educate patients and families.
• Collaborate with physicians and the interdisciplinary care team.
