Job summary

Signature Healthcare at Home is seeking a compassionate Registered Nurse (RN) to join our Home Health team serving the Oregon Coast communities—Lincoln City, Newport, Tillamook, Florence and surrounding areas.

In this role you will provide one-on-one in-home care, build meaningful relationships with patients and families, and collaborate with an interdisciplinary team in a flexible, autonomous schedule.

Qualifications

• Current Oregon Registered Nurse (RN) license.
• Graduate of an accredited nursing program.
• Current CPR certification or ability to obtain.
• Valid driver's license with reliable transportation and automobile insurance.
• Home Health experience preferred but not required.

Responsibilities

• Complete comprehensive patient assessments and develop individualized plans of care.
• Deliver skilled nursing services per physician orders and agency standards.
• Monitor patient conditions and coordinate care with the interdisciplinary team.
• Educate patients, caregivers, and families on disease management and self-care.
• Identify changes in patient status and intervene appropriately.
• Ensure accurate and timely clinical documentation.
• Participate in case conferences and care coordination activities.
• Promote patient safety, independence, and quality of life.
