Job summary

Holly Nursing Care Center in Colorado is seeking a skilled MDS Specialist who will assist the Director of MDS in developing and transmitting resident assessments in line with state requirements and facility goals. The role requires an RN with Colorado license, CPR certification, 1–3 years of direct care experience, and preferably MDS experience in a skilled nursing setting.

You will collaborate across teams to ensure timely, accurate care planning and compliance with regulations.

Qualifications

  • 1-3 years' experience providing direct care to multiple patients/residents.
  • 1 year's previous MDS experience in Skilled Nursing environment preferred
  • Must possess current, unencumbered RN license to practice as an RN in Colorado
  • Current valid CPR certification
  • Knowledge of clinical standards of practice, regulations and reimbursement governing long term care and Medicare participation

  • Participates in development and completion of preliminary and comprehensive resident assessment (MDS) and Care Plan in accordance with current rules, regulations, and guidelines, including the implementation of CAAs and Triggers and Plan of Care.
  • Ensures that care plan includes measurable objectives and timetables to meet resident's medical, nursing, mental and psychosocial needs, that appropriate health professionals are involved in assessment, and that schedule of activities is developed as required.

POSITION SUMMARY

Assists the Director of MDS in the development, completion and transmission of the resident assessment in accordance with the state requirements and policies and goals of the facility.

KNOWLEDGE, SKILLS, ABILITIES & QUALIFICATIONS
  • 1-3 years' experience providing direct care to multiple patients/residents, one years' previous MDS experience in Skilled Nursing environment preferred
  • Nursing degree from accredited college or graduation from approved RN program
  • Must possess current, unencumbered license to practice as an RN in Colorado
  • Current valid CPR certification
  • Knowledge of clinical standards of practice, regulations and reimbursement governing long term care and Medicare participation
