Position Summary The Care Manager is responsible for the development, maintenance and implementation of the standards of nursing practice and objectives as they relate to Minimum Data Set (MDS) requirements, in compliance with federal, state and local regulations. The Care Manager is responsible for optimal reimbursement and proper documentation/diagnoses to justify the need for skilled services and reimbursement requests.

Essential Functions
  • Coordinates and collaborates with clinical staff and therapy for proper documentation, care programs and optimal reimbursement throughout the stay of a skilled resident.
  • Researches all pertinent medical information/diagnoses for optimal reimbursement.
  • Meets regulations for timely certifications and re-certifications and signing of all orders.
  • Meets all requirements in working with Managed Care organizations.
  • Participates in daily meetings with therapy and line staff on progress of skilled residents.
  • Monitors skilled nursing documentation and provides ongoing education as needed.
  • Participates in the development and implementation of the resident assessment process following MDS requirements in accordance with company policies and procedures.
  • Responsible for proper ADL documentation and ongoing education/training.
  • Audits and analyzes MDS documents for accuracy and appropriate completion dates.
  • Ensures timely submission of all MDSs.
  • Oversees and schedules resident care planning meetings.
  • Provides assistance on the floor and on-call/manager on duty as needed.
  • Participates in and attends all required in-service training and education sessions as scheduled.
  • Other duties as assigned.

Physical Demands, Work Environment, and Hours

This position is very active and requires frequent standing, walking, bending, kneeling, stooping, and climbing. Lifts/carries up to 50 lbs. and can push up to 250 lbs. Regular long hours and night/weekend work may be required. Use proper body mechanics to assist residents in daily living. Occasional travel may be required for training or company functions.

Education and Experience Requirements
  • Four-year college or university degree desired.
  • Current state licensure as a Registered Nurse (RN).
  • Training in Medical Records regulations desired.
  • Understanding of diagnosis coding.
  • Working knowledge of prescription drugs, therapy practices and bio-psycho-social sciences.
  • Remain in good standing with all standards of the Nurse Practice Act.
  • Two years of experience in a similar healthcare position preferred.
Knowledge, Skills, and Abilities
  • Ability to read, write, speak and understand English fluently.
  • Ability to meet or exceed the company’s attendance and punctuality standards.
  • Ability to use electronic records and standard office software/equipment.
  • Ability to understand and follow directions and work with minimal supervision.
