Job summary

TEEMA Solutions Group is seeking a Registered Nurse specialized in Minimum Data Set (MDS) to coordinate and complete MDS assessments, ensuring accurate documentation and regulatory compliance for resident care.

You will collaborate with the interdisciplinary team to develop individualized care plans, review coding for reimbursement, and maintain timely MDS submissions in accordance with federal and state guidelines. Experience with LTC, MDS guidelines, CMS policies preferred.

Qualifications

• RN license in state of practice is required.
• BLS certification is preferred or required per facility policy.
• Recent MDS, LTC, or SNF experience is preferred.
• Working knowledge of RAI/MDS guidelines and Medicare/Medicaid regulations is preferred.

Responsibilities

• Complete and coordinate MDS assessments in accordance with federal and state regulations.
• Ensure accurate and timely MDS documentation and submission.
• Develop and update resident care plans with the interdisciplinary team.
• Review clinical documentation to support coding accuracy and reimbursement.
• Monitor resident assessments and communicate changes in condition.
• Maintain compliance with CMS, Medicare, Medicaid, and facility policies.
• Educate and collaborate with nursing staff regarding MDS and care planning requirements.
• Maintain accurate clinical documentation and resident records.

Registered Nurse – Minimum Data Set (MDS RN)

The MDS Registered Nurse (MDS RN) is responsible for coordinating and completing the Minimum Data Set (MDS) assessment process to ensure accurate resident assessments, regulatory compliance, and appropriate reimbursement.
