Job summary

Paycom - ATS is seeking an RN to coordinate the Resident Assessment Instrument (RAI), including the MDS, CAAs, and Care Plans, ensuring compliance with state and federal requirements.

The role requires an active RN license and collaboration with the Interdisciplinary Team to manage skilled services. Additional nursing duties may be assigned as needed.

Qualifications

• Must hold a valid RN license.
• Experience coordinating care plans and MDS/RAI processes preferred.

Responsibilities

• Coordinate and complete the Resident Assessment Instrument (RAI), including MDS/CAAs and Care Plans.
• Collaborate with the facility Interdisciplinary Team (IDT) to ensure compliance with State and Federal requirements.
• Perform other nursing duties as assigned by supervisor.

PURPOSE OF JOB POSITION:The primary purpose of this position is to conduct and coordinate the development and completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Are Assessments (CAAs) and Care Plan in accordance with State and Federal requirements. The MDS Coordinator participates as part of the facility Interdisciplinary Team (IDT) in the systems and processes to manage patients receiving skilled services as assigned.
