Job summary

Apploi is seeking an experienced MDS/RAI Coordinator to oversee the facility’s MDS assessments and the development of comprehensive care plans. You will coordinate an interdisciplinary care plan team, schedule meetings, and audit records to ensure accuracy and compliance with SNF PPS and quality reporting requirements.

The role involves educating leadership and staff, collaborating with health plans and physicians to secure approvals, and interpreting regulatory guidelines to resolve coverage

Qualifications

• MDS/RAI knowledge or training preferred.
• Experience coordinating interdisciplinary care teams a plus.
• Familiarity with SNF PPS and quality reporting programs beneficial.

Responsibilities

• Coordinate the facility’s Resident Assessment Instrument (RAI) process.
• Ensure accurate completion of all MDS assessments and supporting documentation.
• Develop and evaluate residents’ comprehensive plans of care.
• Schedule and lead interdisciplinary care plan team meetings.
• Audit medical records for documentation supporting coded items.
• Educate department heads, physicians, and staff as needed.
• Collaborate with health plans and physicians to secure approvals and ensure eligibility and coverage.

Job description

Coordination of the facility’s Resident Assessment Instrument (RAI) process in accordance with state and federal

Accurate completion of all MDS assessments and any supporting assessments or clinical documentation.

Implementation and ongoing evaluation of each resident’s comprehensive plan of care.

Scheduling and leading interdisciplinary care plan team meetings.

Auditing medical records for the presence of supporting documentation for all items coded on the MDS. Provide

education to department heads, physicians, and other staff as needed.

Collaborating with health plans, physicians and the interdisciplinary team for assuring insurance approvals and/or that

residents meet eligibility and coverage criteria.

Interpreting rules, regulations and coverage guidelines and acting as primary resource for problem solving in regards
to the SNF prospective payment system and quality reporting program.
