Job summary

The Center at Cordera is seeking a full-time RN with MDS/RAI experience to join our care team in Colorado Springs. The MDS Coordinator will lead the development and completion of MDS, CAAs, and Care Plans in alignment with state and federal requirements, working closely with the Interdisciplinary Team to drive timely assessments and accurate documentation.

Responsibilities include ARD planning, MDS completion, CAA processing, and staff education on Nursing ADL documentation.

Qualifications

• RN licensed nurse with experience in skilled or long-term care.
• Experience with RAI/MDS processes and certification preferred.
• Ability to coordinate an interdisciplinary team and communicate clearly.

Responsibilities

• Coordinate MDS/CAAs and care plans with the IDT.
• Set and communicate ARDs and MDS timelines.
• Ensure documentation supports MDS coding in medical records.
• Provide teaching on MDS item completion to staff and CNAs.
• Participate in utilization review and state case-mix management.

We are looking for a FT RN MDS to join our Team.Job descriptionPURPOSE OF JOB POSITION:The primary purpose of this position is to conduct and coordinate the development and completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Are Assessments (CAAs) and Care Plan in accordance with State and Federal requirements. The MDS Coordinator participates as part of the facility Interdisciplinary Team (IDT) in the systems and processes to manage patients receiving skilled services as assigned. Performs other nursing duties as assigned.ESSENTIAL FUNCTIONS:· Coordinates the interdisciplinary team in timely completion of the assessments· Utilizes MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs)· Communicates ARD selection for specific MDSs to IDT· Completes the MDS sections as assigned· Completes CAAs as required· Completes Care Plan items and attends Care Plan meetings as assigned· Monitors overall the documentation in the medical record to validate that it supports MDS coding· Participates in skilled utilization and State Case Mix management as assigned· Provides teaching and training for MDS item completion to interdisciplinary team members that have responsibility for MDS item completion· Provides teaching and mentoring of C.N.A. staff along with Director of Nurses and Staff Development related to completion of the Nursing ADL documentationOTHER DUTIES:· Performs nursing duties as assignedEDUCATION REQUIREMENTSRN· At least two years’ experience working in a skilled or long term care setting· Experience with RAI process and MDS certification preferred''Work RemotelyNo
