Westwood Post-Acute in the Bay Area is seeking an MDS Coordinator who will coordinate MDS completion, collaborate with the inter-disciplinary team, and develop individualized care plans that meet regulatory requirements.

The role emphasizes accurate documentation, CMS guidelines, and resident rights, with responsibilities spanning assessments, plan updates, and ensuring timely MDS submissions. RN license in CA, 1 year LTC/NF experience, and BLS are required.

Qualifications

• RN license active in California.
• Minimum one year of experience in long-term geriatric nursing care or SNF.
• Experience with Minimum Data Set (MDS) assessments preferred.
• Excellent communication and customer service with residents, families, staff, and external agencies.
• HIPAA compliance required.
• Active BLS through the American Heart Association.

Responsibilities

• Coordinate completion of the MDS and participate in inter-disciplinary care planning.
• Develop, evaluate, and update care plans in compliance with regulations.
• Complete MDS within required timeframes.
• Ensure all resident care needs are included in the plan of care.
• Communicate policies and procedures to nursing staff.
• Maintain knowledge of county, state, and federal nursing regulations.
• Stay updated on industry regulations and advancements in geriatric nursing.
• Coordinate MDS assessments with reimbursement processes.
• Work with the Business Office Manager on Medicare, Medicaid, and HMO billing.
