Job summary

Sweetwater Care in California seeks an experienced MDS nurse to support facilities in NorCal (Stockton and Sacramento). The role focuses on leading MDS processes, quality improvement, and reimbursement compliance across the organization.

You will provide education to MDS Coordinators, review assessments for coding accuracy, and help facilities achieve 5-Star quality ratings through QAPI participation and timely RAI completion.

Qualifications

• Must hold an active RN license in the state.
• 3 years’ RN experience with supervisory duties preferred.
• Experience with MDS completion and processes.
• RAC Certification preferred.
• Understanding of EHR systems and related technology.

Responsibilities

• Oversee facility-based resident assessment processes per state and federal rules.
• Audit MDS assessments and supporting documentation for accuracy.
• Review medical records for documentation supporting coded items; educate as needed.
• Interpret regulations and support problem solving for care-based reimbursement and quality reporting.
• Provide consultative training to MDS Coordinators to coordinate resident-centered care.
• Assist Interdisciplinary Team with Care Planning.
• Guide coding related to the Resident Assessment Instrument (RAI).
• Support facilities to achieve 5-Star Quality Measure ratings.
• Monitor RAI timeliness and data accuracy per regulatory standards.
• Identify trends to optimize quality of care and perform Root-Cause-Analysis.
• Review data to ensure accurate diagnosis coding per PDPM.
• Provide ongoing education and support for quality of care delivery.
• Attend and support Quality Assurance Performance Improvement Programs (QAPI).
• Advocate for MDS Coordinators with facility leadership and IDT.
