Job summary

Feather River Care Center seeks an experienced MDS nurse to support NorCal facilities in Stockton and Sacramento. The consultant will lead MDS processes, ensure documentation aligns with PDPM and RAI guidelines, and provide education across teams to improve care quality and 5-Star measures.

You will audit assessments, review records, and train MDS coordinators and IDT members, contributing to survey readiness and regulatory compliance nationwide.

Qualifications

• Current, active RN license in this state.
• Minimum 3 years RN experience with supervisory experience preferred.
• Experience with MDS completion and process.
• RAC Certification preferred.
• Understanding of EHR systems.

Responsibilities

• Overseeing facilities’ MDS assessments in line with state and federal regulations.
• Auditing MDS completion and supporting documentation.
• Reviewing medical records for documentation of MDS items; provide education as needed.
• Interpreting rules and earning potential care-based reimbursement knowledge; act as problem solver.
• Provide consultative training to MDS Coordinators within facilities to coordinate resident-centered care.
• Assist Interdisciplinary Team with Care Planning and PDPM/RAI guidance.
• Educate on clinical documentation and coding related to RAI and PDPM.
• Support facilities to achieve and maintain 5-Star Quality Measure ratings.
• Audit RAI timeliness and accuracy to meet regulatory standards.
• Identify trends, develop processes, and present root-cause analyses.
