Job summary

Sweetwater Care is seeking an exceptional MDS Resource nurse to support Montana facilities. The role leads organization-wide quality and performance initiatives, evaluating facility Quality Measurement and Performance Improvement programs to ensure optimal patient/resident outcomes and appropriate utilization.

The candidate will implement and evaluate strategic quality improvement efforts, collaborate with clinical leadership, and ensure compliance with accreditation and CMS requirements.

Qualifications

• Graduate from an accredited school of nursing.
• Registered nurse with active licensure in the state of practice.
• 3 years of experience in nursing administration as Director of Nursing.
• 5 years of experience in long-term care facilities or an equivalent combination of education and experience.
• 3–5 years of leadership experience in healthcare (preferred).

Responsibilities

• Directs clinical operations for assigned facilities; interfaces with regional support staff and facility teams.
• Possesses the ability to perform the functions of a Director of Nursing and may temporarily fulfill that role if vacancies arise.
• Conducts facility reviews including mock survey visits to assess regulatory compliance and company standards.
• Drives quality improvement and performance management through evidence-based practices and system redesign.
• Translates strategies into objectives, allocates resources, and drives accountability for performance.
• Collaborates with senior clinical leaders to define initiatives for clinical effectiveness and patient safety.
• Facilitates patient safety activities and coordinates multidisciplinary improvement teams.
• Develops policies and processes for quality improvement and performance management.
• Leads mock survey activity and ensures readiness for accreditation and CMS requirements.
• Coordinates with external regulatory and accrediting bodies as liaison for quality improvement.
