Westview is seeking an experienced MDS Coordinator to join our team in Houston, TX. The role focuses on coordinating resident assessments, ensuring regulatory compliance, and maintaining accurate MDS documentation.

Ideal candidates bring MDS/PPS and PSSR experience, fluency with MDS 3.0, and familiarity with long-term care Medicaid workflows. Proficiency with MDS software such as Point Click Care is required.

Qualifications

• Must have 2 years of long term care experience.
• MDS/Tile experience required and MDS 3.0 fluency.
• Proficiency with MDS software and regulatory guidelines.

Responsibilities

• Coordinate resident assessments including admission, annual and quarterly reviews.
• Ensure regulatory compliance with Federal/State MDS guidelines.
• Monitor MDS and care plan documentation for accuracy.
• Maintain MDS software data entry and reporting accuracy.
• Lead interdisciplinary meetings for Medicaid and care planning.
• Update care plans based on current assessments.
• Generate Quality Indicators reports for leadership.
• Submit timely MDS data per standards.
• Attend professional development to stay current.
• Oversee ancillary data for billing reporting.
