Red Oak Nursing & Rehab in Texas is seeking an experienced MDS/PPS Coordinator to join our team. LVN or RN with MDS/PPS and PSSR experience is preferred. The role oversees resident assessments, regulatory compliance, MDS data submission, and care plan updates to support high-quality resident care.

Qualifications

• Must have 2 years of long term care experience.
• Must have MDS / TILE experience.
• Must be fluent in MDS 3.0.

Responsibilities

• Coordinate resident assessments including admission, annual, quarterly, Medicare, and change-of-condition assessments with timely communication with nursing and departments.
• Ensure regulatory compliance with Federal and State guidelines for MDS.
• Review MDS and care plan files for accuracy and organization.
• Maintain MDS software proficiency to ensure correct data entry and reporting.
• Lead interdisciplinary meetings and coordinate Medicaid, ITTM, and care plan discussions.
• Update and revise care plans based on current assessments and resident status.
• Generate and distribute Quality Indicators Reports to leadership.
• Submit MDS data timely per reporting standards.
• Stay current with industry standards through professional development.
• Oversee ancillary data and report to corporate billing for accurate processing.
