Stonegate is seeking an experienced MDS/PPS Coordinator to join our team in Fort Worth, TX. The role requires LVN or RN licensure and deep MDS/PPS experience, including Medicaid, MDS and Point Click Care knowledge.

You will coordinate resident assessments, ensure regulatory compliance, review documentation, and lead interdisciplinary meetings to support care quality. Competitive compensation and benefits offered.

Qualifications

• Must have 2 years of long term care experience.
• MDS / TILE experience required.
• Must be fluent in MDS 3.0.
• Must possess a current, unencumbered RN or LVN license in the state.

Responsibilities

• Coordinate Resident Assessments across admission, annual, quarterly, Medicare, and change-of-condition assessments.
• Ensure timely communication with nursing and other departments.
• Follow Federal and State guidelines for comprehensive assessments (MDS).
• Review MDS and care plan files for accuracy and organization.
• Maintain expertise in MDS software to ensure correct data entry and reporting.
• Attend and coordinate Medicaid, ITTM, and care plan meetings.
• Update care plans based on current assessments and resident status.
• Generate and distribute Quality Indicators Reports to leadership.
• Submit MDS data timely per reporting standards.
• Stay current with professional development in the field.
• Oversee ancillary data reporting to corporate billing.

MDS COORDINATOR

In this role, you will:

• Coordinate Resident Assessments: Oversee admission, annual, quarterly, Medicare, and change-of-condition assessments, while ensuring timely communication with nursing and other departments.
• Ensure Regulatory Compliance: Follow all Federal and State guidelines for comprehensive assessments (MDS).
• Monitor Documentation: Review MDS and care plan files for accuracy, thoroughness, and organization.
• Utilize MDS Software: Maintain expertise in software systems to ensure correct data entry and reporting.
• Lead Interdisciplinary Meetings: Attend and coordinate Medicaid, ITTM, and care plan meetings for smooth team communication.
• Update Care Plans: Adjust and revise care plans as needed based on current assessments and resident status.
• Provide Quality Reporting: Generate and distribute Quality Indicators Reports to leadership to support care improvement.
• Submit MDS Data: Ensure timely and accurate transmission of MDS assessments per reporting standards.
• Stay Current: Attend professional development sessions to keep up with industry standards and best practices.
• Oversee Ancillary Data: Collect and report ancillary services to corporate billing for accurate processing.

Knowledge / Experience

• Must be fluent in MDS 3.0

Licensure / Certifications

• Must possess a current, unencumbered, active license to practice as a Registered Nurse or Licensed Vocational Nurse in this state.
