Meadows @ Cypress Wood in Texas seeks an experienced MDS/PPS Coordinator to join our team. The ideal candidate will have an RN or LVN license, MDS/PPS experience, PSSR experience, and fluency with MDS 3.0.

You will coordinate comprehensive assessments, maintain regulatory compliance, update care plans, and lead interdisciplinary meetings for smooth resident care and accurate reporting. This role requires attention to detail and strong collaboration with clinical and billing teams.

Qualifications

• Must have 2 years of long-term care experience.
• Must have MDS / TILE experience.
• Must be fluent in MDS 3.0.

Responsibilities

• Coordinate resident assessments (admission, annual, quarterly, Medicare, change-of-condition) and communicate with nursing and other departments.
• Ensure regulatory compliance with Federal and State guidelines for MDS assessments.
• Review MDS and care plan files for accuracy and organization.
• Maintain expertise in MDS software and data entry/reporting.
• Attend and coordinate Medicaid, ITTM, and care plan meetings for team communication.
• Update care plans based on current assessments and resident status.
• Generate and distribute Quality Indicators Reports to leadership.
• Submit MDS data per reporting standards.
• Stay current with professional development in the field.
• Oversee ancillary data collection/reporting to corporate billing.

MDS COORDINATOR

In this role, you will:

• Coordinate Resident Assessments: Oversee admission, annual, quarterly, Medicare, and change-of-condition assessments, while ensuring timely communication with nursing and other departments.
• Ensure Regulatory Compliance: Follow all Federal and State guidelines for comprehensive assessments (MDS).
• Monitor Documentation: Review MDS and care plan files for accuracy, thoroughness, and organization.
• Utilize MDS Software: Maintain expertise in software systems to ensure correct data entry and reporting.
• Lead Interdisciplinary Meetings: Attend and coordinate Medicaid, ITTM, and care plan meetings for smooth team communication.
• Update Care Plans: Adjust and revise care plans as needed based on current assessments and resident status.
• Provide Quality Reporting: Generate and distribute Quality Indicators Reports to leadership to support care improvement.
• Submit MDS Data: Ensure timely and accurate transmission of MDS assessments per reporting standards.
• Stay Current: Attend professional development sessions to keep up with industry standards and best practices.
• Oversee Ancillary Data: Collect and report ancillary services to corporate billing for accurate processing.

Knowledge / Experience:

• Must have 2 years of long term care experience
• Must have MDS / TILE experience
• Must be fluent in MDS 3.0

Licensure / Certifications:

• Must possess a current, unencumbered, active license to practice as a Registered Nurse or Licensed Vocational Nurse in this state.
